PATIENT INFORMATION NOTICE (CLARIFICATION TEXT)

 

1. DATA CONTROLLER

This information notice has been prepared by Sıla Physical Therapy and Rehabilitation Center, acting in the capacity of the Data Controller, in accordance with Article 10 of the Law No. 6698 on the Protection of Personal Data (KVKK) and the Communiqué on the Procedures and Principles to be Followed in Fulfillment of the Obligation to Inform.

 

2. PURPOSES OF PROCESSING PERSONAL DATA, CATEGORIES OF PERSONAL DATA TO BE PROCESSED, LEGAL GROUNDS FOR PROCESSING, AND METHOD OF COLLECTION OF PERSONAL DATA

 

The purposes of processing your personal data, the categories of personal data processed, and the legal grounds for processing are comparatively presented below.

Your personal data is processed through manual (non-automated) methods, based on the legal grounds stated above, through physical submission and completion of the relevant forms.

 

3. TO WHOM AND FOR WHAT PURPOSE PERSONAL DATA MAY BE TRANSFERRED

Your personal data may be transferred by the Clinic for the following purposes:

  • Providing information to authorized persons, institutions, and organizations,

  • Conducting advertising / promotional activities,

  • Conducting financial and accounting activities,

to authorized public institutions and organizations, as well as to real persons or private legal entities with whom we have contractual relationships. In the event that your explicit consent is obtained, your personal data may also be shared publicly on the website / social media platforms and within the scope of scientific activities.

 

4. RIGHTS OF THE DATA SUBJECT

You may submit your requests within the scope of Article 11 of the Law, which regulates the rights of the data subject, to Sıla Physical Therapy and Rehabilitation Center, in accordance with the Communiqué on the Procedures and Principles of Application to the Data Controller, in writing to the following address:
