This Information Notice has been prepared by Sıla Physical Therapy and Rehabilitation Center (hereinafter referred to as the “Clinic”), acting in the capacity of the Data Controller, in accordance with Article 10 of the Law No. 6698 on the Protection of Personal Data (KVKK) and the Communiqué on the Procedures and Principles to be Followed in Fulfillment of the Obligation to Inform.

Your personal data processed by the Clinic is not transferred to any third parties.
4. RIGHTS OF THE DATA SUBJECT
You may submit your requests within the scope of Article 11 of the Law, which regulates the rights of the data subject, to Sıla Physical Therapy and Rehabilitation Center, in accordance with the Communiqué on the Procedures and Principles of Application to the Data Controller, in writing to the following address:
